That's why at Janis Chiropractic and Wellness Center our aim is to  remove any  stress or causing unnecessary tension and to make you feel comfortable. In this area of the website you will be able to take care of items necessary for your visit prior to arriving, in the comfort of your own home.

This portal will allow you to: 

-Fill out your online paperwork
-View your upcoming appointments
-See your appointment history
-Edit your contact information  and
-Change your access to email notifications and text reminders, all with an easy click. 

We use this system for ALL NECESSARY INTAKE FORMS needed for appointments and updated history, as well as being more integrated with your care.

If you are unable to fill out forms via: a phone, computer, tablet, etc...please inform the front desk before booking your appointment.
